Medicare Therapy Coverage For Lymphedema

Getting Medicare to recognize lymphedema therapy as essential medical expenditure has been a difficult task. Lymphedema patients have been long trying to get the benefit of Medicare for the expenses that they incur for the therapy that continues throughout their life. The Women’s Health and Cancer Rights Act states that complications arising due to breast cancer surgery, which includes lymphedema, must be covered by all insurance providers. However, this coverage does not extend to those who suffer from primary or secondary lymphedema. In February 2008, the compression garments considered an essential part of lymphedema therapy were classified as covered items for Medicare.

Lymphedema is a condition that has no medical cure, although it can be managed with therapy which is known as the Complete Decongestive Therapy or CDT. The process involves a lymphatic massage combined with the use of compression bandages and compression garments, a skin care routine and a regular exercise regimen. The most important aspect of the therapy is the manual lymph drainage (MLD) which aims at draining the stagnated lymph to reduce the swelling. Occasionally, the therapist might use the Sequential Gradient Pump to loosen fibrotic tissues before the massage. The therapist performing the lymphatic massage is a specialized professional trained in the technique. Therapy sessions in the early stages may be held frequently, at least five days a week. Those who do not have easy access to the lymphedema therapist may use a sequential gradient pump for the lymphatic drainage process.

Medicare covers the lymphedema pump therapy, but recently the coverage rules have been modified. Previously, although the pneumatic compression devices were included in coverage, the patient had to attempt all other methods of treatment first, a process that took many months. This has changed now. Compression devices have been included under durable medical expenses for both primary and secondary lymphedema. There is a four-week trial period which the doctor must observe. A course of medication, use of compression garments and elevation of the limb is followed by the patient and if no improvement is seen, the doctor prescribes a pneumatic pump. The doctor is required to give a Certificate of Medical Necessity to enable the patient to procure a lymphedema pump from a Medicare authorized supplier. The supplier of the pump must be enrolled with Medicare and possess the Medicare Supplier Number, or you will not be reimbursed the claim.

In case of compression garments, which can be a large recurring expense, lymphedema sufferers have had to deal with legislatures which prevented their inclusion in Medicare coverage lists. A positive judgment in February 2008 has come to the aid of lymphedema patients. Compression garments were classified as items meeting the standards for covered items. Items like compression bandages, compression sleeves and stockings were considered medically essential and would be covered as prosthetic devices under Medicare for lymphedema therapy. The lymphedema patients can take advantage of this edict and claim a compensation for these medically essential items that help them to manage the lymphedema.

Before starting any work use a plastic cover to protect the surfaces of the vehicle from damage. Then separate trim parts from the packing material. The actual installation begins with by test fitting each trim piece to its proper location. This allows you to check the fit of each piece as well as to familiarize yourself with its mounting location. At this time you will want to know the easiest way to properly align each part.

Check for the most obvious straight lines such as switches, vent or radio cuts out for a correct positioning. Because the wood trim is flat you will need to note areas which will require special attention if the need a bending to match sharp curves. Remember you’re installing real wood and you want to make sure it’s flexible enough before test fitting to an area. Therefore we recommend that if the temperature is below 80 degrees Fahrenheit you should use a heat gun or hair drier or the vehicle’s heating system to raise temperature to between 80 and 100 degrees on the trim components. Use a low heat setting of the heat gun or hair drier and continuously moving with a back and forth motions to prevent an overheating and damaging the surfaces. The heat gun or hair drier should be 6-7 inches from the surface of the wood. However, if you are using a home type hair drier, you will need to head wood trim longer to reach the desired flexibility. It is very important to properly clean all the factory surfaces, to which the trim parts would be applied. DO NOT use the sharp fabrics for cleaning this factory surfaces or panels. Instead use soft towels and the special wipes provided. Do NOT reuse towels on the cleaning steps as they become contaminated. Apply 3M Prep-Sol 70 to a fresh towel to clean the factory surfaces. Then wipe the surfaces dry with a clean towel.

DO NOT spray or pour the cleaner directly on the factory surfaces. If the vehicle’s surfaces were protected by special covers, you will need to repeat the cleaning process for 5 times. An easy test to check your cleaning process is to stick a piece of masking tape to a clean surface. If the tape does not properly stick, repeat the cleaning steps. When the interior surfaces are properly cleaned with 3M Prep-Sol 70, use the alcohol napkins to thoroughly wipe down the surfaces then allow them to air dry. Now you will again fit each trim piece paying close attention to the operational switches, vents, ash trays, cup holders and so on. When each individual part is properly positioned, trace around the outside edge with the pencil. This will mark the area to be treated with adhesion promoter. Carefully apply the adhesion promoter to the areas in the trace line. If any promoter was accidentally applied to the areas not to be covered with new trim parts, you must immediately wipe it off before it dries. Use a clean towel wet with alcohol and carefully wipe the area. DO NOT apply extra pressure as it could remove color from the factory panel. When the promoter has dried for 3-4 minutes you are ready to begin the application of the trim parts. Carefully remove the protecting backing tape from the trim pieces. Then heat both sides of the wood trim and the factory surface to which the trim will be applied. On larger parts you will need to pull off the section of the backing, position the piece, and then remove the rest of the backing as you laid the piece to its place.

Be careful not to touch the adhesive backing of the panels. When you are heating the trim components and the factory surfaces between 80 and 100 degrees, remember to use a low heat setting and keep the heat gun or hair drier moving at all time. It is VERY important to align each part before application because once installed, removing the particular part may damage it or the surface it was adhered to. Carefully hold the trim piece at a slight angle to the surface it will be mounted on. Starting with one edge properly align the piece to its position as you lay it down. Check the proper operation of the switches, vents and other moving parts. Press the trim firmly at the place, using your finger tips. Pay the particular attention for the parameter of each piece. Then press the whole part firmly with a clean towel to ensure proper adhesion. With all the new trim parts in place you now need to recheck the operation of all moving parts of the interior. Check them up and down and side to side movements of all vents. Check the proper operation of the window and door lock switches and all other moving parts. Then remove finger tips and smudges from the panels with a dry and scratch resistant fabric. The installation is now complete. Place the warranty card in a glow box and impress everybody with your newly upgraded vehicle.

All About Microsoft Access 2013 and Access 365

Microsoft Access 2013 is the software that has progressed over the ages from Access 1.1 in 1992 until now.

Office Professional 2013 Software (which includes Word, Excel, PowerPoint, Outlook and Access) will allow you to install and run Access 2013 as usual.

The Office 365 version is paid by monthly subscription and provides Web services to Office 2013. It includes the Office Professional 2013 software as well as Access 365 and Access 2013.

Access 365, besides the name “Access”, has little to nothing in common with Access 2013 or previous versions of Access.

What is Access 365 all about?

Access 365 is a tool for creating web apps that run within SharePoint 2013. The old .ACCDB format has been abandoned. New applications should use the Access 365 model, or stick with Access 2010.

Access 365 allows the rapid creation of small applications using the standard web technologies of HTML5, CSS and JavaScript in a simplified web development environment. One big feature is that Access Services can now store data to Microsoft SQL Server or an SQL Azure database.

The use of SQL Server overcomes the 2 Gigabyte size limitation of the previous Access databases. SQL Server also provides all the security and features essential for any strategic company business system.

And What about Access 2013?

The Access 2013 desktop version is much unchanged from previous versions – it still supports the Front-End and Back-End database model and Visual Basic for Applications (VBA).

But Access 2013 no longer supports:

  • The Microsoft Access project (ADP)
  • The Access 97.mdb database format
  • PivotCharts and PivotTables
  • The dBASE database
  • Smart Tags (Action Tags)
  • Access 2003 Toolbars and menus
  • The Microsoft SQL Server database Upsizing Wizard
  • The Developer Source Code Control

Apps for Office

There is a new feature of Access 365 – Apps for Office. This technology enables Office to be used as a service on the Cloud or Web. The webpage can be hosted in an Office application – that is Excel, Word, Outlook, PowerPoint or Project.

An app is usually published to the Office Store for use.

JavaScript API for Office

Access 365 has a new JavaScript API that provides programmatic access to Microsoft Office documents. The API includes application-specific data types, objects, functions and events.

The JavaScript API is a great improvement on the crude JavaScript design capabilities within HTML.

SharePoint apps

Access 365 provides a simple way for SharePoint to host the Front-End of an app and have the data management capabilities of SQL Server on the Web. This simplifies web development.

Note that Visual Basic for Applications (VBA) code is not compatible with SharePoint Access applications.

Access 365 Macro Editor

Templates can be used to create sophisticated looking apps – and without the need for code. However business rules and data manipulation can be implemented with a new macro language. The macros do not have the power of VBA, but are adequate for simple logic.

Microsoft’s Strategic Direction

Microsoft Access was never meant for corporate company databases. Many companies evolved the Access database software into their core administration systems. But with the limitations of network traffic, peak loads, reliability, rollback recovery, security, etc, etc – these systems eventually hit a brick wall.

The direction that Access 365 is now taking reverts to the original intention of Microsoft Access. That is, providing a rapid development environment for small Line of Business (LOB) applications.

Microsoft Access with VBA is at the end of the road. For mission critical administration systems, the best development platform is Visual Basic.Net with SQL Server.

Writing and Revising Your Life Story

Change is not simple. Why do we repeat behavior that doesn’t work? Those actions that lead to stifling debt, disappointing careers, or stuck relationships? Then do it harder, yet expect a different result? Why is it not obvious that trying to exit an old story by simply writing a “better ending” only recreates the same story, and ensures that we remain in it? That a thousand better endings to an old story don’t create a new story? That the past cannot be changed and is a settled matter? That too often, we see ourselves as the victims of the stories that we author and the feelings we create?

We actively construct what we think, feel, and experience.

How surprised we are to learn that our fears are not in the dim shadows of the past’s unknown, but in the hopeful light of this moment’s change.

The only thing more difficult than changing and growing is not doing it.

It is never too late to become what you might have been. Or too soon to become who you want to be.

As adults, we are the sole authors of our own life stories. Every day begins a fresh page. The dramas of everyday life do not simply affect us, they are created by us. Yet so often the story closest to us, our own, is the most difficult to read How can we tell our life stories to ourselves in order to know which aspects of the narrative work and which need to change? How can we identify what is missing, change an attitude, or generate happiness? How can we shift our understanding to see life not as a multiple-choice test with certain predetermined answers, but as an open-ended essay question?

12 STEPS TO LISTEN TO YOUR LIFE STORY

This exercise intends to illuminate invisible decisions camouflaged as beliefs and assumptions. This exercise intends to align your efforts with a refocused vision.

1. Crystallize awareness of beliefs, views, and opinions that you hold in each area of your life: family, business, personal, financial, creativity, and spiritual development. Recognize that none of these are facts, but beliefs that are created. The beliefs, points of view, and opinions are decisions that you make, a perception that you hold.

You can track when in time you made your original decision that led to the view or belief that is limiting. Most often the original decision arises from disappointment, or what you did not get. An example is a decision to be cautious about relationships, and protect yourself in case of rejection. This belief brings about what you fear, though perhaps based originally on adaptive protection from physical or emotional abandonment. Problems are not written into your genes, though an assumption such as victimhood can be a powerfully organizing storyline, even an aspect of identity.

2. Look for the link/connection between the original decision to the view or perspective held now. Acknowledge the impact it has on your current life, the costs, and the exchanges that you make. Does each belief serve you right now?

At one time, the decision served you but you may have outgrown it. Is it still worth the cost that you pay? Are you exchanging valuable time and energy in pursuit of something that ultimately is disappointing?

3. Try new perspectives and possibilities. You have to try on and live an experience to get informed data of how it may bring a change to your life. An experiment may be an idea or image that you live into, and evolve it to create a habit.

4. Explore what is possible. From the place of what is possible, clarify what you want to create, and what action would be paired with it.

5. Recognize and honor your uniqueness. Your uniqueness includes distinctive capacities and abilities, what you do exceptionally well, what works best for you. The design of your life plan must recognize your exceptional strengths, and place your energy on leveraging strengths, rather than creating obstacles. Are you engaging your passion and creativity to do what you do uniquely well in your life and career?

6. Recognize that which you can determine, and that which you cannot. Let go/accept what you cannot determine rather than engaging it with hope and ultimate frustration. Embrace that which benefits you and the elements that serve you, and let go of all that do not.

7. Do only that which works in current time and that is consistent with your needs and values. The bottom line of any theory or belief system is: Does it work now?

8. Clarify decisions about how you use, invest, and refurbish your life energy based on your life plan.

9. Change is a process, not an event. Design short-term, step-wise measurable goals to validate your progress. Hold yourself accountable to the timetable of your goals.

10. Review your tolerations list. Update and revise it.

11. Create a mission that is stronger than your fear.

Focus your energy on where you are: the present– and where you are headed: the future. You cannot change the past, but you can free yourself from its grips.

Winning Roulette Against All Odds

Charles DeVille Wells was said to be "The Man Who Broke the Bank in Monte Carlo." An enduring mystique has grown around this story even inspiring a song about it. It was also said that he accomplished this feat using the "Labouchere" or Cancellation System. It's hard to say how much truth there is to the legend, but I do know this is an insidious little system that seems to win on balance until a long series of unfavorable decisions creates a catastrophic loss. Wells died broke in 1926.

Roulette is one of the simplest of all casino games but holds a remarkable fascination for it's followers. The crudest recorded versions date back to the ancient Greeks and Romans. The modern game we know today began in France in the 1800's. The name Roulette is a French word meaning "little wheel."

In England a variation of the game was called EO (stood for even / odd) and in the 1800's America's wild west saloons they played another variation of a wheel game called Faro.

The European version of roulette is designed with 36 numbers and a single 0. With an odds payout of 35 to 1 the house percentage is vastly more favorable than the double zero wheel that is used on this side of the Atlantic.

With the single zero wheel and a feature called "en prison," the house percentage is 2.7%. When the zero appears, all wagers on the outside of the table (odd / even, red / black, high / low) are not lost and stay on the table for the next spin or the player has the option to forfeit half his bet. These features reduce the casino's percentage to 2.7% and on even money wagers it further reduced to 1.35%.

Roulette in American casinos employs a zero / double zero on their wheel and is a world apart from the European ones. Using the 0/00 added to the 36 numbers increases the casino's advantage to 5.26% which is why the game enjoys much greater popularity in European casinos than it does here in the US The payoff on winning bets are the same, 35 to 1, the Negative house percentage is double.

Despite the increased house edge against the player, there are still some advantages to playing roulette. First, it can be a very relaxing game and provides a nice diversion from some of the more serious table games. It can be an enjoyable way to pass the time.

Secondly, since it is a game with a low expectation of a winning income, it can be played with very low table minimums or starting capital.

This is not to say that it is impossible for the intelligent player to win playing roulette. On the contrast, compared to wagering on slot machines or the BIG 6 wheel where the percentages range from 10% on certain machines, to as high as 18% against the player it is by far a better bet.

Although I regularly hear the "experts" say that the roulette wheel can not be beaten, that the player is guaranteed to lose, I can not help but to disagree. Losses do occur at times, but on balance, I have been a net winner my entire life. If it can not be beaten, how is that possible?

In the 40 years I have spent gambling in casinos I have experienced with a wide variety of playing and betting methods. Some of these methods were marginal at best, but some that I've used were excellent performers. The long odds payoffs on the inside layout (the numbers) combined with a disciplined wagering system have the ability to produce exceptional results.

Although I agree that in the "long run" no system can overcome a negative expectation, it is in the "short run" that the player must exercise the discipline to always quit when winning.

The game lends itself nicely to even money wagering on the outside of the table layout and most "up as you win" wagering systems tend to also be profitable when combined with reasonable expectations.

My problem with the "experts" regarding roulette is that they look at the negative house percentage and quickly dismiss it as a game that can not be beaten. Their real life casino gambling seems to be either very limited or possibly nonexistent. My own personal experience is in sharp contrast.

The player's advantages are clear. The low capital requirements make it ideal as a low risk venue. The pace is leisurely and the rules uncomplicated with low table minimums. There are playing strategies that will, in the short run, give the player the opportunity for very attractive gains.

These are a few of the rules that must be followed to be successful playing roulette.

1. Familiarize yourself with the correct playing and betting systems.
2. Do not vary from these playing methods by betting on hunches or riskier wagers.
3. Decide on the amount of capital you will risk and do not exceed that amount. If you lose that amount have the discipline to walk away.
4. Bet minimum amounts when losing and only increase your bet size after a winning wager. Never increase your bet size to try and recoup losses … ever.
5. Keep your playing sessions short.
6. The game of roulette was not designed to favor the player …
Always quit when you are winning!
7. Do not gamble with money you can not afford to lose.

Greedy players are seldom if ever winning players. Do not try to "break the bank." It can not be done. But, with good judgment and a fair amount of discipline, roulette can be can be both enjoyable and very profitable. Play to win.

Caroline Forbes

Caroline Forbes is the best friend of Elena Gilbert and Bonnie Bennet. They use to share everything together. The trio also goes to the same high school and even in the same class in Mystic Falls. Caroline's mother is the sheriff of Mystic Falls and her father is outside town. Caroline seems to be a wonderful girl, that means she does not really care about others. She always wants to have the best looking girl and does everything to achieve it. This is how the people who do not know her well would describe her. But for her friends is Caroline a lovely person that in fact does care for her friends. Even Stefan realizes that after some time and does compare her to his best friend Lexi.

Caroline got converted to a vampire by Katherine Pierce. Caroline's mother, Elizabeth Forbes, could not handle this situation. As she has been hunting vampires her lifelong and now her daughter is a vampire too. After some time of not speaking together they finally met and talked. And now they are like a family again. Any as new personality in the world of the vampires, Caroline remains loyal towards humans. Stefan teachers her how to control her new powers and with his help Caroline does not have to fear anymore that she could kill anyone by mistake.

Caroline & Damon:

Everything started when the Salvatore brothers came back to Mystic Falls. Damon already had an eye on Caroline since he saw her for the first time. They met the first time in a bar, where Caroline falls in love with him quickly. She invites Damon to go home with her and already in their first night of being together he shows her, his real face. He starts to compel Caroline and uses her as his private blood supply. Of course is Caroline scared and tries to fight him. Without having any chances to win that fight, she has to keep doing what Damon tells her. After a while of playing with her, Stefan talks to Damon and he can figure out a way to stop Damon using Caroline.

Caroline & Matt:

One day in the Episode "162 Candles" they feel something between each other and start a love story. In the beginning of the second season the things start to get weird between them as Matt is a human and Caroline is a vampire now. Caroline has to stop the relationship because she is scared of that she could hurt Matt. She does not have her powers under control yet. Matt can not life without her and tries to get her back. Caroline is still in love with him but in the end she tells him directly that they can not be together because she could kill him accidentally.

Caroline & Tyler:

Caroline never liked Tyler in the first place. But when Tyler became a werewolf they started to hang around a lot together. Caroline was helping him with his problem and was there for Tyler. As he did not know anything about vampires yet, Caroline told him she is the only vampire in town and because of that she could help and understand him with his transformation problem. Later on a few other werewolves came in the town and they took Caroline as a host and tortured her to threat Stefan and Damon. Tyler did not know any of that but still as soon as Caroline was free again, she quit the friendship with Tyler and he left Mystic Falls. Later on Caroline was together with Matt. The relationship did not hold that long and when they broke up Tyler was there to console Caroline. By that time they came together.
